Patient Athena Portal

PacWest Healthcare utilizes Athena EHR for documentation of patient care which allows patients to access to the information below.

The features on the Patient Information Center include:

• My Health page

• Patients can view, download, and transmit their health record, which includes:

  • Patient name

  • Demographic information (preferred language, sex, race, ethnicity, date of birth)

  • Provider's name and office contact information

  • Current and past problem list

  • Procedures

  • Current medication list and medication history

  • Current medication allergy list and medication allergy history

  • Height, weight, blood pressure, BMI

  • Smoking status

  • Care plan fields, including goals and instructions

  • Known care team members, including the primary care provider (PCP) of record

 

Note: The electronic Patient Care Summaries appear on the My Health page.

To obtain your patient login ID you will need to provide an email address to our PacWest Administration team at officeadmin1@pacwest.com.  Our team will complete the registration process and you will receive your password to login.

Your email will serve as your Login ID. Use the link below to access your medical records once the above process is complete.
